For those monitoring what our professional associations are doing during this tumultuous time, here's this from AALL:


AALL SIGNS ON TO JOINT LETTER TO PROTECT ACCESS TO GOVERNMENT INFORMATION

Yesterday, the American Association of Law Libraries (AALL) partnered with OpenTheGovernment.org and more than 60 other public interest groups and associations on a letter to the Office of Management and Budget (OMB), asking OMB to issue guidance reminding agencies that they are required under the Paperwork Reduction Act (44 U.S.C. 3501 et seq.) to give adequate public notice when making significant changes to information on their websites, including when information is taken down temporarily or removed. The letter comes after weeks of concern expressed by AALL and other organizations about data and other information no longer being accessible on government websites.

The letter stated:

It is crucial that agencies comply with the law, so that the public does not lose access to vital government information that helps them protect themselves and hold the marketplace and their government accountable. Failure to give adequate notice of removal could mean that years of government work on important issues are effectively lost to the public. Moreover, providing the required notice will assure compliance with the law, smooth transition issues, ensure public participation, calm fears of censorship, and will reduce litigation costs for both the government and the public interest community.

"AALL has a long-standing commitment to ensuring that governments provide reliable access to information. Our Principles and Core Values Concerning Public Information on Government Websites clearly states our belief that information published on government websites should be accessible, comprehensive, trustworthy, and preserved," said AALL President Ronald E. Wheeler Jr. AALL will continue to advocate for agencies' compliance with laws governing access and preservation, and to ensure that government information remains accessible to all.

The health plans covering public school and state employees in 2015 spent an average of about 31 percent more on obese employees or their obese spouses than they did on others, a report found.

.

According to the report by the Arkansas Center for Health Improvement, the plans spent an average of $4,302 in 2015 on health expenses for employees and spouses whose reported height and weight indicated they were obese, compared with an average of $3,270 on slimmer employees and spouses.

.

The average health costs were even higher for employees and spouses who said they exercised fewer than 20 minutes per week -- $6,043, compared with about $3,445 for those who exercised more.

.

The study analyzed the results of a survey employees and spouses were required to fill out in 2015 to avoid having an extra $75 per month added to their premiums the next year.

.

The report was prepared for the State and Public School Life and Health Insurance Board, which governs the health plans.

.

Chris Howlett, director of the Department of Finance and Administration's Employee Benefits Division, said he expects the board to begin considering options soon for encouraging healthy behavior among employees.

.

For instance, he said, the plans could discount premiums for employees who maintain a healthy lifestyle or meet goals such as quitting smoking or losing weight.

.

"I know there's definitely an interest in individuals wanting to put, instead of cost increases across the board, cost increases to the population that becomes noncompliant" with health-related requirements, Howlett said.

.

The plans cover about 45,000 public school employees and about 26,000 state employees, in addition to retirees and the spouses and dependents of retirees and employees.

.

This year, the plans' premiums rose an average of just 2 percent for public school employees and 3 percent for state employees.

.

But the board's actuarial consultant has warned that larger increases will be needed in future years unless medical and drug costs rise more slowly or state and school district funding increases.

(Reuters Health) - Exercise may be an efficient way for obese pregnant women to lower their risk of diabetes, dangerously high blood pressure and other complications, research suggests.

The study suggests that a prenatal exercise-based intervention leads to both decreased costs and improved outcomes in obese women, said Leah Savitsky, a medical student at Oregon Health and Science University in Portland who led the study.

As reported at the 37th annual meeting of the Society for Maternal-Fetal Medicine in Las Vegas, Nevada, Savitsky and her team analyzed previously published research on the effect of exercise on pregnant women with a body mass index (BMI) of at least 30.

Based on their analyses of those earlier studies, the researchers estimated the benefit of exercise for women who are obese at the start of their pregnancy.

They calculated that for every one million such women, there would be 38,176 cases of a dangerous pregnancy-related blood pressure problem known as preeclampsia among those who exercised, compared to 113,000 cases among those who didnt exercise.

.

Exercise would also be linked to a decrease in so-called gestational diabetes, with a rate of 195,520 per million among exercisers compared to 305,500 among non-exercisers, according to their calculations.

.

Likewise, they estimated, for every million obese pregnant women the preterm birth rate would drop from 105,059 to 90,923 with exercise, the maternal death rate would fall from 90 to 70 and the neonatal death rate would drop from 1,932 to 1,795.

.

Based on a cost-effectiveness threshold of $100,000 per quality-adjusted life year, an exercise intervention could save money as long as expenses are held to just under $3,000.

While diet, exercise and behavior modification are essential components of obesity management, a successful long-term weight loss strategy should also include avoiding or minimizing medication-related weight gain, according to a new report from Weill Cornell Medicine.

.

In the paper, published Feb. 10 in Gastroenterology, investigators from the Comprehensive Weight Control Center at Weill Cornell Medicine underscore the necessity for physicians to evaluate the potential side effects and interactions of medications they prescribe for patients with obesity, and to familiarize themselves with alternatives that may limit weight gain or may even assist with weight loss.

"Physicians know that some medications can increase weight, but they don't always know what alternatives are out there," said lead author Dr. Leon Igel, an assistant professor of clinical medicine in the Division of Endocrinology, Diabetes and Metabolism at Weill Cornell Medicine. "We want physicians who treat patients with obesity to feel more comfortable prescribing these alternatives. Our paper looks at how to practically manage patients with obesity by prescribing the optimal medications for them."

However, a patient's weight loss desire does not typically guide a physician's prescribing practices. "Each practitioner has a goal in mind," Dr. Igel said. "A cardiologist wants to lower blood pressure. A psychiatrist wants to make sure that mood is well regulated. They're not necessarily focusing on which medications will affect weight, but rather what will achieve their treatment goal."

It is essential, therefore, that internists and other physicians be aware of what medications their patients are taking and how they are likely to interact; recognizing when they can prescribe one medication as a substitute for another, or pair a medication that causes weight gain with one that minimizes its effect. "Not everyone can be taken off insulin," Dr. Igel said. But some patients might be able to couple insulin with other agents that promote weight loss to neutralize insulin's effect on weight.

Everyone is continually talking about getting physically active and how to
stay physically active. But you dont hear a lot of talk about how to
recover from strenuous physical activity.  It doesnt matter if the workout
is for a sport, training for a marathon, or just your daily exercise
routine. We all need to recover from our activities to help lessen the
muscle soreness and joint pain that comes along with a tough workout.
There are many ways to recover from a workout. Here are a few of the best
ways to help your body recover.

People who eat diets high in sugar may be at an increased risk of Alzheimers disease, a new study has warned.

.

For the first time a tipping point molecular link between the blood sugar glucose and Alzheimers disease has been established by scientists from the University of Bath and Kings College London in the UK.

.

They have shown that excess glucose damages a vital enzyme involved with inflammation response to the early stages of Alzheimers. Abnormally high blood sugar levels, or hyperglycaemia, is well-known as a characteristic of diabetes and obesity, but its link to Alzheimers disease is less familiar.

Diabetes patients have an increased risk of developing Alzheimers disease compared to healthy individuals.

By studying brain samples from people with and without Alzheimers using a sensitive technique to detect glycation, the team discovered that in the early stages of Alzheimers glycation damages an enzyme called MIF (macrophage migration inhibitory factor) which plays a role in immune response and insulin regulation.

MIF is involved in the response of brain cells called glia to the build-up of abnormal proteins in the brain during Alzheimers disease, and the researchers believe that inhibition and reduction of MIF activity caused by glycation could be the tipping point in disease progression.

It appears that as Alzheimers progresses, glycation of these enzymes increases.

Normally MIF would be part of the immune response to the build-up of abnormal proteins in the brain, and we think that because sugar damage reduces some MIF functions and completely inhibits others that this could be a tipping point that allows Alzheimers to develop, said van den Elsen.

Abstract

Glucose and glucose metabolites are able to adversely modify proteins through a non-enzymatic reaction called glycation, which is associated with the pathology of Alzheimers Disease (AD) and is a characteristic of the hyperglycaemia induced by diabetes. However, the precise protein glycation profile that characterises AD is poorly defined and the molecular link between hyperglycaemia and AD is unknown. In this study, we define an early glycation profile of human brain using fluorescent phenylboronate gel electrophoresis and identify early glycation and oxidation of macrophage migration inhibitory factor (MIF) in AD brain. This modification inhibits MIF enzyme activity and ability to stimulate glial cells. MIF is involved in immune response and insulin regulation, hyperglycaemia, oxidative stress and glycation are all implicated in AD. Our study indicates that glucose modified and oxidised MIF could be a molecular link between hyperglycaemia and the dysregulation of the innate immune system in AD.

Over the past year, I have been involved in countless discussions and debates about whether obesity should be declared a chronic disease (as it has been by numerous medical organizations). Therefore, I thought it might be helpful to review the common arguments made by those on either side of the debate.

.

Arguments Against Labeling Obesity a Disease

.

1. BMI is not a good measure of health.

.

This is perhaps the most common argument made against calling obesity a disease, and I have long railed against the use of BMI (body mass index) as a clinical definition of obesity, as it is neither a direct measure of body fatness nor does it directly measure health. In fact, its specificity and sensitivity to pick up health problems associated with obesity (such as type 2 diabetes or hypertension) are so limited, it would not meet the criteria commonly applied to most other diagnostic tests.

So, if not BMI, what should be used as the defining characteristic of obesity?

.

The World Health Organization (WHO) defines obesity as "...abnormal or excessive fat accumulation that may impair health."

.

2. The relationship between body fat and health is inconsistent.

.

It is true that the impact of body fat on a person's health depends on a range of factors, from a genetic predisposition to the type of body fat. Some would argue that such variability refutes a causal relationship between fat and health.

.

The direct impact of excess body fat is most evident when the amount of fat directly impedes physical functioning. It may be harder to understand the relationship between excess or abnormal body fat and metabolic problems.

3. Obesity is modifiable and preventable.

.

This argument is true for many other conditions that are accepted as diseases, including stroke and heart disease. Most strokes and the vast majority of heart attacks are both preventable and modifiable (once they occur), as are diabetes, osteoarthritis, obstructive lung disease, and many forms of cancer.

.

4. Labeling it a disease will detract from obesity prevention.

.

In no other instance has calling something a "disease" stopped us from doing the utmost to prevent it. Consider efforts to prevent heart disease (dietary recommendations, fitness, smoking cessation), cancers (physical activity, healthy diets, smoking cessation, sunlight exposure), or infectious diseases (vaccinations, food safety, hand washing, condom use).

.

In fact, if one embraces the concept that established obesity is a life-long problem for which we have no curethe very definition of "chronic disease"we should be redoubling our efforts at prevention. Governments, organizations, and individuals should be more committed to preventing a "real" disease that has become an epidemic.

.

5. The "disease" label would reduce personal responsibility.

.

Let's consider type 2 diabetes, another avoidable and modifiable condition. Calling diabetes a disease does not reduce the individual's ability to prevent it or to change the course of the disease. And what about heart disease or lung disease, or even cancer? We still expect patients to help manage these diseases.

.

6. The "disease" label stigmatizes people living with obesity.

.

Obesity is already a highly stigmatized condition, as are other diseases such as depression and HIV/AIDS. Refusing to call obesity a disease will not address this problem. We must help people understand the complex and multifactorial nature of this disorder and the rather limited treatment options that we currently have.

.

In fact, it is the fairytale of "choice" and the overly simplistic "eat less, move more" propaganda that promote discriminatory stereotypes and the notion that people with obesity are simply not smart or motivated enough to change their slovenly ways.

7. The "disease" label essentially medicalizes a behavior.

.

The underlying assumption here is that the root cause of obesity is a behavior, which may be true on the most superficial level. Yes, behaviors such as eating too much and being too sedentary can promote weight gain. But nowhere in the WHO definition of obesity is there any mention of behavior.

.

Many people understand that the relationship between behavior and weight gain is not straightforward.

.

8. The "disease" label promotes helplessness and hopelessness.

.

There are many people living with chronic diseases (eg, diabetes, hypertension) who are well controlled, who do just fine with treatment, and who go on to live long and productive lives.

.

We do need better treatments for obesity, but even now, people living with obesity can change the course of their disease (often with professional help) by identifying and addressing the root causes of their weight gain (eg, depression, emotional eating) and by adopting behaviors which, even if not resulting in any noticeable weight loss, can markedly improve their health and well-being.[3]

9. Obesity is just a risk factor for disease, not a disease itself.

.

This is only true if one (wrongly) considers elevated BMI as the definition of obesity, because no doubt, people with higher BMI levels carry a higher risk for obesity-related complications such as type 2 diabetes, sleep apnea, fatty liver disease, and hypertension.

.

However, when you use the WHO definition of obesitynamely, "accumulation of excess or abnormal fat that impairs health"obesity is far more than just a risk factor. Using that definition, a person with a BMI of 35 may be at risk of developing obesity (but may not yet have it); only when their excess fat starts impairing their health does it become a disease in its own right.

.

Even then, one might argue that obesity itself is not the disease, but rather the complications of obesity are the real diseases.

.

This notion is both right and wrong.

.

10. Obesity affects too many people to be characterized as a disease.

.

Some critics have warned that declaring obesity a disease would instantly turn millions of people into "patients," overwhelming the healthcare system. I hear from payers and policy makers that providing medical treatments for obesity is simply not practical because of the number of people who have it.

.

That didn't stop us from calling diabetes a disease, or depression, or the flu, all of which affect millions of people.

.

In fact, even if a disease affected 100% of the population, we would not shy away from calling it a disease.
