Well the rufflers themselves actually have moving parts so they will make a noise especially as when fitted they are attached to the needle bar which goes up and down.

I don't know what the directions say about the Janome ruffler but I believe any metal attachment with moving parts needs occasional oiling to make it work to it's best - just one drop at a time though and then work the attachment to get the oil to the necessary points.
