03R/W bank lists

After squinting myself silly for so many years following the eccentric
presentation of the bank lists in the manual, I have finally transcribed
them into a single document in three parallel columns, each in simple
numeric sequence.

I am happy to provide the pdf to anyone who gives me the means of doing so.
