I use Free Youtube-to-Mp3 converter
from Videosoft. It is an accessible program, and in the advanced options
section is a checkbox to make it "screen-reader" friendly.

It does at the end when you close the
program. Just ignore it and hit alt-f4 again!
It does a great job, lets you easily
choose the format you want to save the audio portion in, and you can choose

For anyone who has installed the program, I can email you a .re file with all my settings. All you have to do is open the file and the settings will automatically get imported. I have mine set to minimize to the system tray, too convert links copied to the clipboard automatically, to convert at the highest bit rate (320k) and to name files with artist and title.
