As I recall, a few years ago the Berks FHS published a response from a retired vicar to a similar question about the accuracy of marriage records. 

He said that the pressures of a wedding day, and the sometimes 'tired and emotional' state of those crammed into the vestry to complete the register, regularly caused major mix-ups in the names and details recorded. 

As a result, he had long adopted the practice of insisting that all those involved should visit the church the day before, and complete the register in a calm and, by implication, more sober manner.  The number of errors dropped dramatically.  Would that more vicars had done the same...
