I don't know if the link will work, but it's a "15 Note DIY Hand Crank Music Box Movement Note DIY Music Punch with 20 Music Papers Music Box Kit" for less than $12.  It's got 20 pages of paper strip, a mini-hole puncher for the strips, and even the music crank-box.  They're all over ebay as well.  You can also buy the paper strips in big rolls.

Seems like this would enable you to standardize the paper strips, and provide a fairly easy way of punching the holes, and even has a printed grid on the paper strips.  Then all you would need to do is figure out the reading mechanics.
