see if this goes through, here is coping tubing prior to welding with a cross drilling fixture on the QCTP.  The shaft is turned using a 1/2" electic drill.   Using my set up there is a severe limit to how long the bit can be.  If the holder is made offset, like I believe Jim's is, then it has more useable drill length.
 
Jim, the drill chuck that was mentioned in 4 was probably not a powered one, but more likely an Aloris 35 or equivalent, where the drill chuck is fixed and is feed by means of the lathes carriage power drive with the work in the chuck spinning and the drill/ream/tap/boring bar etc fixed as it would be in the tailstock.
