Connect motor/encoder cables to controller
Remove cover from controller
Go to "Test and Tune" in ServoConfig and put motor in manual mode
Move the slider to start motor turning
Check voltage

Dear Don,
I exchanged the cable from another motor on declination axis to test. The problem is still there. Therefore, the problem should be the encoder.
I prefer replacing the whole motor assembly

Are you mean that inputting 5volt to pin1 and pin4 by say a power supply? Then check the voltage change on multi-meter while rotating its shaft?

You can check if the encoder is working with voltmeter.
Make sure no static conditions present
Remove encoder cover
Supply only 5 volts to pin 4 of encoder and ground to pin 1.
Turn encoder

Yes, I have tested all the contacts with multi-meter. Confirmed that from the contacts on the encoder to the contacts on the controller are ok.

You really should try a new cable, sometimes a wire and pin connection can be lost when plugged into controller.

This problem indicates no encoder signal to controller.
Change the motor/encoder cable with a cable you know is good. Most problems are in the modular plugs of cables
