Ed,

  The negative, for the small capacitor, is also negative for the entire board.  If you are familiar with the negative output of the bridge rectifier, you can test with a meter to be sure.   That is where you can solder the stay alive's black with white stripe wire.

  The blue wire is soldered to the +12V contact (opposite side of the motor contacts).
