Steve -- When someone uploads a file, there is a "notify members" checkbox in the upload dialog. However, the person doing the upload has to opt in...there is no group setting to force such notifications, nor is there a corresponding checkbox for the creation of a database table.

Does this sound familiar as I don’t recall how to do this or is it a simple case of “wishful thinking”?
It appears to be the latter.  :-)

But, is there a Junk or Spam folder in the webmail interface, and can you create a filter that moves a message to the INBOX?
Yes; there's a Spam Box, and I found a solution late yesterday evening (UK time!) Buried in BT Mail's labyrinthine "Help" section I spotted something I think I had seen once some time ago but not followed up. Incoming senders are checked with addresses in the recipient's (webmail) Contact List and if they are there then it assumes that the incoming traffic is legitimate. I set it up last night and it seemed to be OK after 12 hours or so (fortunately the group in question is quite busy at the moment) so I gradually migrated incoming material from a temporary moderatorship (new word!) that I had set up on a second membership that goes nowhere near BT Mail. Fingers crossed; so far group traffic is not being interrupted at all, although it was a bit annoying to find an email from someone else was shunted into the spam box for no obvious reason.

So for now anyway the matter has been resolved. (Famous Last Words!) FWIW the "Help Pages" are so convoluted and cross - linked I greatly doubt if I could ever find the relevant section again. It's a wilderness of mirrors in there...

People who sign up to a group need to demonstrate that they
   1) actually own that email address they provided (after clicking the "Join This Group" button), and
   2) are capable of (and actually wish to) receive groups.io messages there.

This is the purpose of the confirmation process; it's not a sign that anything is broken.

Those who already have a confirmed groups.io account -- perhaps by having previously subscribed to another group, or having gone through a Yahoo transfer, or who click on "Join This Group" while already logged in to groups.io -- do not need to have it done again.

Is there a fix to prevent this from happening to other potential members?
Not without paying for a Premium group. See https://groups.io/g/GroupManagersForum/wiki/Status-Badges-in-the-Members-List

UPDATE: It seems the problem was the subscriber's "Confirm Your Groups.io Subscription" email going to spam folder.

Just out of curiosity as I have never used the email subscribing on groups.io

I just did a test on one of my groups with an unused email address

It worked flawlessly and in seconds

A suggestion that may help some folks: switch to digests. Not a solution if you like the interaction of receiving and sending emails all day, but for folks, like me, who spend an hour on groups in the morning and then leave them be for the rest of the day, digests reduce the volume of email and the odds that it will be classed as spam. When they are classed as spam, it's only one email to report not spam.
Best, Marv

Is the email address you're sending from already subscribed to the group?

If so, nothing will happen...not even an error message.

Thanks but at the bottom of this member's page there is no Confirm button.
You must have a Basic group. Only Premium and above can confirm members without their intervention.

The buttons displayed are Save/ Send message/ Remove/ Ban/ Send Confirmation Email
Check the box next to the person with the NC bade and select Send Confirmation Email from the Actions menu. The subscriber will have to respond to this email to remove the badge.

23.3.3 Confirming members who are marked as Not Confirmed
In a Premium or Enterprise group, owners and moderators can confirm members who requested to join the group but have not yet responded to the confirmation message sent by Groups.io. These members are marked with a Not Confirmed   badge in the member list.

To confirm a member who is marked as Not Confirmed:
1. Display the membership record of the member whose membership you want to confirm.
2. Go to the bottom of the member’s page and click the Confirm button.
Note: In a restricted Premium or Enterprise group, if you approve a pending member who is not yet
confirmed, the approval also confirms their membership automatically.

You can then copy or move messages to folders that are local on your computer.
Depending on how your client is set up and operates, it may also be used to rearrange the items on the server.  I use Thunderbird and Gmail mostly, so when I move a message from the Gmail Spam folder to the Gmail Inbox in TB, it also moves it on the server.  I resisted IMAP for years, but don't think I could get along without it now.  Mainly because I use a desktop at home and a laptop while traveling.  Everything stays in sync automatically and I don't have to remember to copy from one to the other.  As with any program, there are details to work out, but it works for me.
