+01: ACCESSIBILITY.

Using Keyboard-Only Navigation, for Web Accessibility

By Joseph C. Dolson.

"Blind and low-vision users, as well as those with mobility
disabilities, rely on their keyboards - not a mouse - to navigate
websites. Online forms are keyboard 'traps' when they don't allow a
user to tab through it without completing a field. That is not the
case with Newegg's checkout form, which properly allows users to tab
through it, and ultimately return to the browser bar, without
completing a field..."

http://www.practicalecommerce.com/articles/ 98316-Using-Keyboard-only-Navigation-for-Web-Accessibility

The Myth of the Keyboard-Only User

By Joseph C. Dolson.

"...Situational or temporary accessibility problems are still
problems, and should be considered of equal importance to any
permanent problem."

https://www.joedolson.com/2016/03/myth-keyboard-user/

Should WCAG.NEXT Require All Functionality Operable With a
Mouse/Pointer (in Addition to Keyboard)

By David MacDonald.

"I inadvertantly created quite a dust up last week when I asked
whether our current personas for sighted keyboard only users are
overly coarse and simplistic..."

http://davidmacd.com/blog/ should-WCAG-require-all-functionality-by-mouse.html

1.2.2 Captions (Prerecorded)

By Rakesh Paladugula.

"Captions are provided for all prerecorded audio content in
synchronized media, except when the media is a media alternative for
text and is clearly labeled as such..."

http://www.maxability.co.in/2016/03/1-2-2-captions-prerecorded/

Testing With the NVDA Screen Reader

By Jonathan Whiting.

"This is a piece about how I configure and use NVDA..."

LMS Accessibility - Is ATAG the Answer?

By Scott Hollier.

"The evolution of Learning Management Systems (LMS) has been a
fascinating one..."

http://www.accessiq.org/news/w3c-column/ 2016/04/lms-accessibility-is-atag-the-answer

Be Wary of Accessibility Guarantees from Vendors

By Adrian Roselli.

"In my ~20 years of responding to RFPs/RFQs, once organizations
started to realize the value of accessibility (or fear of lawsuits), I
saw more and more requests include a note on accessibility. In most
cases this was just a single line item among many, often with nothing
more than a checkbox: 'Is your solution Section 508 compliant?'..."

http://adrianroselli.com/2016/03/ be-wary-of-accessibility-guarantees-from-vendors.html

15 Things to Know About Twitter's Alternative Text for Images

By Deborah Edwards-Onoro.

"...Like many other Twitter users, I'm thrilled to have the option to
add alternative text to images in Twitter. It makes my favorite social
media channel even better. A more accessible product means more people
can enjoy Twitter. That's a win-win for everyone! And I can't wait for
it to be released for the web version as well as other Twitter
third-party apps."

https://www.lireo.com/15-things-to-know-twitter-alternative-text-images/

What You Should Know About Accessibility + SEO, Part I: An Intro

By Laura Lippay.

"...It's a challenge many of us don't think about every day, but some
of what we do in SEO can affect the experience that people with visual
impairments have when visiting a page..."

https://moz.com/blog/accessibility-seo-1

Power Tool Retailer Hammers Law Firm Behind ADA Web Accessibility Suits

By Alexis Kramer.

"Over 50 complaints have been filed since the beginning of 2015
against well-known retailers for allegedly failing to make their
websites accessible to the blind..."

http://www.bna.com/power-tool-retailer-b57982069062/

+02: CASCADING STYLE SHEETS.

CSS Containment

By Michael Scharnagl.

"If you build simple sites without tons of DOM nodes and widgets you
probably don't have to care about CSS containment. On the other hand,
if you build more complex sites CSS containment will help to optimize
the performance. It is also a good idea to set contain: strict; for
third-party widgets to protect the performance of your site."

https://justmarkup.com/log/2016/04/css-containment/

+03: COLOR.

Color Safe: Build Accessible Color Palettes Based on WCAG Guidelines

By Sarah Gooding.

"...Color Safe is a handy web app that helps designers select color
combinations that will meet WCAG 2.0 guidelines for those with
different visual capabilities..."

http://wptavern.com/ color-safe-build-accessible-color-palettes-based-on-wcag-guidelines

Why Your Gray Text Should Never Exceed 46% Brightness

By UX Movement.

"The color gray comes in many different shades. You can find them on
different elements across most sites. Dark gray is often used for
headings and body text. Light gray is often used for metadata, labels,
and descriptions. But the shade that’s abused the most is light gray."

http://uxmovement.com/content/ why-your-gray-text-should-never-exceed-46-brightness/

+04: EVALUATION & TESTING.

6 Reasons Not to Test with Users (and Why to Test Anyway)

By Jess Hutton.

"...In this article, we'll respond to all of these objections, and
give UX practitioners the ammunition they need to convince any
doubters that usability testing is, in fact, right for every
project..."

http://www.uxbooth.com/articles/ 6-reasons-not-to-test-with-users-and-why-to-test-anyway/

5 Reasons Why Your First User Research Activity
Should be a Usability Test

By David Travis.

"A usability test is the wrong research method when you want to
discover if there's a real user need for your product; when you want
to understand the environment where your system is used; and when you
want to find out how people use your product in their daily lives. So
why do I almost always recommend a usability test as a team's first
user research activity?"

Custom Element Semantics

By Steve Faulkner.

"The following content written by me, was until recently a part of the
W3C Custom Elements specification, but was removed/substantially
re-written when a new editor took over. As we think, in the form
represented here, it's a useful introduction to custom element
semantics, we have re-published it..."

https://www.paciellogroup.com/blog/2016/04/custom-element-semantics/

+07: NAVIGATION.

The Hamburger Menu and the Zombie Apocalypse

By Gerry McGovern.

"Print is dead but its zombies have come back to ruin our lives with
hamburger menus: grey, meaningless text and huge, meaningless images.
Opening soon in a theatre of the absurd near you: 'Revenge of the
Brochureware Designers' and 'Nightmare on PDF Street: The Sequel.'"

http://www.gerrymcgovern.com/new-thinking/ hamburger-menu-and-zombie-apocalypse

+08: TOOLS.

Introducing Outline Audit

By Hugo Giraudel.

"We recently had some minor issues in a project where the final
document outline on some pages was somewhat broken. I investigated a
little, and ended up building a tiny tool to audit the document
outline. More on that in a bit, but first..."

careless design..."

+10: XML.

A Working SVG Workflow for Accessible Icons

By Hugo Giraudel. (Hat tip to Jennifer Sutton)

"It has been commonly admitted that using fonts for icons is a bad
practice. It’s bad for rendering (sizing, positioning, etc.), it has
weird failures (proxy browsers, CORS, etc.), it conveys poor
semantics, can hurt accessibility, and the list goes on..."

Better to be Approximately Right than Exactly Wrong

consider whether you're holding out for the perfect metric or method
to get the perfect result. Exactness on the wrong question is not the

If you are sitting in a Grade 10 classroom in Ontario studying civics this year, you may well be using Civics and Citizenship, published by Oxford University Press. Its a standard text for the compulsory course, examining how the Canadian political system works and asking students to consider such questions as whether our justice system is just or whether, if you didnt live in Canada, you would want to. It was written and edited by Canadians and also offers online access to an interactive workbook and a directory of 250 primary and secondary sources.

.

Released in 2014, it is the last schoolbook Oxford will publish in Canada. The venerable academic publisher, a non-profit institution owned by the British university, which has operated a Canadian branch since 1904, stopped producing texts for Canadian schools two years ago, cancelling plans for history and geography texts that were in the works. Oxford still sells its backlist to schools but will not consider publishing new school titles nor refreshing old ones.

.

As learning goes digital, publishing textbooks has become an increasingly difficult business everywhere, but Canada has made the situation worse, threatening childrens access to Canadian teaching materials, through its ill-conceived copyright legislation.

.

Balancing users rights with those of creators is always a tricky business and when the previous Conservative government updated the copyright law in 2012, publishers and writers warned that it had made far too large and vague an exemption for educational purposes, considering that digital copying in universities and photocopying in schools is standard practice and was licensed to the education sector by publishers. At the time, provincial education ministries promised they would only copy what was fair, but defining fair has proved highly contentious.

.

In 2013, emboldened by the new environment, the education ministries (outside Quebec) stopped making payments to Access Copyright, which licences copying in schools and universities for the publishers. Alberta had sued the association and in 2012 got a decision from the Supreme Court, ruling that the copying of short passages, even if copied multiple times, was fair. The court, which was split 5-4, did not define short but the Council of Ministers of Education has, telling teachers they can freely photocopy as much as 10 per cent of a work or one chapter in any book.

Study: Medical Errors are the Third Biggest Cause of Death in America

Updated by Sarah Kliff

.

.

Medical errors kill more people than car crashes or new disease outbreaks. They kill more people annually than breast cancer, AIDS, plane crashes, or drug overdoses. A new study estimates that they are the third leading cause of death in the United States, causing a quarter-million fatalities in 2013 alone.

.

Patient safety researchers Marty Makary and Michael Daniel published new data in the British Medical Journal Tuesday suggesting that preventable medical errors resulted in 251,454 deaths in 2013. If that estimate is correct, the only bigger causes of death are heart disease and cancer.

.

The researchers worry, however, that their number is actually an underestimate that medical harm kills even more patients than we're currently able to count.

.

"Something like 2 to 3 percent of people who go into the hospital are going to have some pretty severe harm as a result," Don Berwick, the Obama administration's former Medicare administrator, previously told Vox. "Australian studies show that the rate might be as high as 12 percent. The harder you look, and the more you study the issue, the more errors you find."

.

It's incredibly difficult to know how many deaths result from medical harm

.

When there is a plane crash in the United States, federal law requires airlines to report that information to the National Transportation Safety Board. It investigates each incident and tries to figure out what went wrong and how it could be prevented in the future.

.

But there's no analogous body in health care.

.

When a patient dies as a result of medical harm, there's no regulator that has to get notified the hospital doesn't send off paperwork about the error that occurred. Sometimes the information gets jotted down in the patient's medical record, but even that is not a certainty.

.

This makes estimating the frequency of medical harm very difficult and researchers generally believe that their figures underestimate the prevalence of harm. Their study uses data from four recent studies, all of which relied on medical records to estimate fatalities caused by medical errors. So the authors know that their estimate of fatalities misses any errors that weren't captured in the medical record.

.

"We believe [our estimate] understates the true incidence of death due to medical error because the studies cited rely on errors extractable in documented health records and include only inpatient deaths," Makary and Daniel write.

.

Still, they argue that there is value to putting out the best number they can find, as it can draw attention to the potential magnitude of a rarely discussed problem in health care.

Elsevier Complaint Shuts Down SCI-HUB Domain Name

Hoping to stop the unauthorized distribution of millions of academic papers, academic publisher Elsevier filed a complaint against Sci-Hub and several related sites last year.

.

While Sci-Hub is nothing like the average pirate site, it is just as illegal according to Elseviers legal team, which obtained a preliminary injunction from a New York District Court last fall.

.

The injunction ordered Sci-Hubs operator to quit offering access to any Elsevier content, but this didnt happen. Instead of taking Sci-Hub down, the lawsuit and the associated media attention only helped the site grow.

.

However, as part of the injunction Elsevier is able to request domain name registrars to suspend Sci-Hubs domain names. This happened to the original .org domain earlier, and a few days ago the Chinese registrar Now.cn appears to have done the same for Sci-hub.io.

.

The domain name has stopped resolving and is now listed as reserved according to the latest WHOIS info. TorrentFreak reached out to Sci-Hub founder Alexandra Elbakyan, who informed us that the registrar sent her a notice referring to a complaint from Elsevier.

.

Elbakyan was also quick to add that several backup domain names are still in play, including Sci-Hub.bz and Sci-Hub.cc. This means that the site remains accessible to those who update their bookmarks.

Sci-Hub

From Wikipedia

.

Sci-Hub is an online repository of over 51,000,000[1] scientific academic papers and articles,[2] available through its website. New papers are uploaded daily after accessing them through .edu proxies.[3] Founded by Alexandra Elbakyan from Kazakhstan in 2011, it began as a reaction to the high cost of research papers behind paywalls, typically US$30 each when bought on a per-paper basis.[4] Academic publisher Elsevier has in 2015 filed a legal complaint in New York City alleging copyright infringement by Sci-Hub.[5]

Sci-Hub is providing science publishers with their Napster moment

A report from Science shows that academic paper piracy site Sci-Hub is not a niche product catering to cheapskates and isolated mad scientists: Its as popular as it is illegal, and its millions of users span the globe, from Tehran to Boston.

.

For those not aware, Sci-Hub is, in the X for Y startup lingo of our days, The Pirate Bay for research. Millions of papers, many of which require a fee or institutional subscription of some kind to access, have been downloaded in full and added to the Sci-Hub database. You can get just about any paper you want with a few keystrokes and, naturally, publishers are furious.

.

Alexandra Elbakyan, the young Kazakh grad student who founded Sci-Hub in 2011, provided Science reporter John Bohannon with six months of usage data which, incidentally, the journal is kindly providing for anyone to download themselves.

.

The stats, which have been scrubbed of personally identifying information, are impressive: There are more than 50 million papers stored, with between 4 and 6 million downloaded a month. There are users on every continent but Antarctica

Paper piracy sparks online debate

Data on Sci-Hub activity prompts discussion about why the research-paper website is so popular.

Piracy site for academic journals playing game of domain-name Whac-A-Mole
Just like The Pirate Bay, Sci-Hub is domain hopping to beat takedown court orders.

We reported a few weeks ago on a popular pirate site for science journals whose oversees admin was being sued by one of the world's leading academic publishers, Elsevier. Elsevier is the same New York publisher that the late Aaron Swartz had noted in his "Guerilla Open Access Manifesto" that told academics and researchers they had a "duty" to free the knowledge they were privileged to read behind Elsevier's paywall.

.

Because of the lawsuit, which Sci-Hub founder Alexandra Elbakyan has refused to participate in, she's been engaged in a game of domain-name Whac-A-Mole in response to Elsevier winning court orders demanding the shuttering of the popular site's domain name. The site allows anybody, not just academics, to access tens of millions of scholastic research articles for free.

US Nuclear Emergency Messaging System Still Uses Floppy Disks

A shorter URL for the above link:

.

.

The Defense Departments system for sending emergency messages to nuclear forces is made up of aging technology that runs on a 1970s-era computer system and uses 8-inch floppy disks.

.

The Government Accountability Office (GAO) made the finding in a broad report highlighting the billions of dollars the government spends every year to maintain largely outdated information technology (IT).

.

Replacement parts for the system are difficult to find because they are now obsolete, the report concluded.

.

The Defense Departments Strategic Automated Command and Control System was one of the most egregious examples highlighted in the GAO report. In a footnote, the GAO had to define floppy disk, noting that modern flash drives hold the equivalent storage of 3.2 million floppy disks.

.

snip

.

The GAO report concluded that agencies across the government are spending far too much on operation and maintenance of aging IT instead of modernization and enhancements.

.

In 2015, $80 billion was spent on government IT. The large majority of it, $61 billion, was spent on operation and maintenance. That share has increased since 2010.

.

The report also found that nearly $1 billion in IT operation and maintenance investment has been labeled as high or moderate risk.

.

.

The complete article may be read at the URL above.
