The Justice Departments efforts to force Apple AAPL +2.35% to help it hack into the iPhone that belonged to one of the San Bernardino shooters may be over, but if recent history is any indication, it wont be long before lawyers for the government are back in front of a judge with a similar request.

The American Civil Liberties Union today published details of 63 cases where the government has cited the All Writs Act to force Apple or Google GOOGL +1.67% to help it gain access to data contained on locked smartphones and tablets. The list includes cases involving drugs, identity theft and counterfeiting, among others. The disclosure comes one day after a judge threw out an order to compel Apple to help investigators in the San Bernardino case by writing software to disable a basic security function on the iPhone 5c. The order was vacated after lawyers for the Justice Department told the court Monday that they had what they needed from the phone and no longer needed Apples help.

We just thought transparency is so important, that we needed to see what these cases were, to see how the arguments evolved. It was unbelievable to us that this issue had been litigated for the most part outside of public view, said Matthew Segal, legal director of the ACLU of Massachusetts. For weeks, the group dug up any available information on cases using public databases, as the collective focus of privacy and security researchers, civil liberties groups, Silicon Valley and the media centered on the San Bernardino case.

Apple disclosed earlier this month a dozen other cases it was compelled to help with under the All Writs Act. Its unclear how many of those are included in the ACLUs list, since Apple did not release case names or docket numbers. Justice Department lawyers said previously that Apple had helped investigators unlock phones at least 70 times since 2008, prompting the ACLU to gather more information on the cases.

ACLU attorneys identified the cases by combing through court records with key words and phrases that appeared in previously identified filings. While no two requests made under the All Writs Act are exactly alike, Segal said there were enough similarities among them to come up with search terms to find others.

The ACLU also filed a Freedom of Information Act request with the Justice Department in December, seeking additional information on every case involving the All Writs Act. The Justice Department informed ACLU that they were gathering responsive documents, but as of today none have been produced, said Riana Pfefferkorn, a fellow at the Stanford Center for Internet and Society, which joined the ACLU in the request. Pfefferkorn said the San Bernardino case should be an indicator of the governments ability to hack into devices in other instances.

Is this just a case of April Fools arriving early, or is Trader Joes actually closing?

Yahoo! Style reported Thursday that Trader Joes will be closing all of its 457 stores by January 2017. Additionally, the grocer would also discontinue all products.

The Yahoo article reported that Trader Joes released a press release Thursday morning, however, there is no announcement posted to the site.

In a statement to WHO-TV, Trader Joes public relations director Alison Mochizuki said:

Although we dont take ourselves too seriously, we dont want anyone to mislead our customers. Trader Joes is open for business and has no plans to close. We had no involvement whatsoever in the April Fools hoax article.

If you are sitting in a Grade 10 classroom in Ontario studying civics this year, you may well be using Civics and Citizenship, published by Oxford University Press. Its a standard text for the compulsory course, examining how the Canadian political system works and asking students to consider such questions as whether our justice system is just or whether, if you didnt live in Canada, you would want to. It was written and edited by Canadians and also offers online access to an interactive workbook and a directory of 250 primary and secondary sources.

Released in 2014, it is the last schoolbook Oxford will publish in Canada. The venerable academic publisher, a non-profit institution owned by the British university, which has operated a Canadian branch since 1904, stopped producing texts for Canadian schools two years ago, cancelling plans for history and geography texts that were in the works. Oxford still sells its backlist to schools but will not consider publishing new school titles nor refreshing old ones.

As learning goes digital, publishing textbooks has become an increasingly difficult business everywhere, but Canada has made the situation worse, threatening childrens access to Canadian teaching materials, through its ill-conceived copyright legislation.

.

Balancing users rights with those of creators is always a tricky business and when the previous Conservative government updated the copyright law in 2012, publishers and writers warned that it had made far too large and vague an exemption for educational purposes, considering that digital copying in universities and photocopying in schools is standard practice and was licensed to the education sector by publishers. At the time, provincial education ministries promised they would only copy what was fair, but defining fair has proved highly contentious.

In 2013, emboldened by the new environment, the education ministries (outside Quebec) stopped making payments to Access Copyright, which licences copying in schools and universities for the publishers. Alberta had sued the association and in 2012 got a decision from the Supreme Court, ruling that the copying of short passages, even if copied multiple times, was fair. The court, which was split 5-4, did not define short but the Council of Ministers of Education has, telling teachers they can freely photocopy as much as 10 per cent of a work or one chapter in any book.

Study: Medical Errors are the Third Biggest Cause of Death in America

Medical errors kill more people than car crashes or new disease outbreaks. They kill more people annually than breast cancer, AIDS, plane crashes, or drug overdoses. A new study estimates that they are the third leading cause of death in the United States, causing a quarter-million fatalities in 2013 alone.

.

Patient safety researchers Marty Makary and Michael Daniel published new data in the British Medical Journal Tuesday suggesting that preventable medical errors resulted in 251,454 deaths in 2013. If that estimate is correct, the only bigger causes of death are heart disease and cancer.

.

The researchers worry, however, that their number is actually an underestimate that medical harm kills even more patients than we're currently able to count.

.

"Something like 2 to 3 percent of people who go into the hospital are going to have some pretty severe harm as a result," Don Berwick, the Obama administration's former Medicare administrator, previously told Vox. "Australian studies show that the rate might be as high as 12 percent. The harder you look, and the more you study the issue, the more errors you find."

.

It's incredibly difficult to know how many deaths result from medical harm

.

When there is a plane crash in the United States, federal law requires airlines to report that information to the National Transportation Safety Board. It investigates each incident and tries to figure out what went wrong and how it could be prevented in the future.

.

But there's no analogous body in health care.

.

When a patient dies as a result of medical harm, there's no regulator that has to get notified the hospital doesn't send off paperwork about the error that occurred. Sometimes the information gets jotted down in the patient's medical record, but even that is not a certainty.

.

This makes estimating the frequency of medical harm very difficult and researchers generally believe that their figures underestimate the prevalence of harm. Their study uses data from four recent studies, all of which relied on medical records to estimate fatalities caused by medical errors. So the authors know that their estimate of fatalities misses any errors that weren't captured in the medical record.

.

"We believe [our estimate] understates the true incidence of death due to medical error because the studies cited rely on errors extractable in documented health records and include only inpatient deaths," Makary and Daniel write.

.

Still, they argue that there is value to putting out the best number they can find, as it can draw attention to the potential magnitude of a rarely discussed problem in health care.
